Physical Examination
Students registering for Athletics must have a current physical examination on file in the Nurses office cleared by the district physician before the student is authorized to participate in practice/tryouts. To ensure clearance for participation, physicals should be completed and returned to Union High School.
PHYSICALS ARE TO BE TURNED IN AT UNION HIGH SCHOOL MONDAY -THURSDAY 8AM-2PM AND FRIDAY FROM 8AM-NOON.

Union Athletics Physical Clearance
The 3 step process
Step 1- Complete the online sports registration form on
Arbiter Registration (FALLS SPORTS ONLY AT THIS TIME)
- Be sure to fill out all sections and acknowledge all forms.
- Full time Union Magnet/ Vo-Tech Students are able to register by using the link above.
ALL ATHLETES MUST HAVE A COMPLETED REGISTRATION ON ARBITER REGISTRATION TO BE CLEARED TO PARTICIPATE- Not having a complete registration in which all forms are complete will delay the process to be cleared.
Step 2- Complete the NJDOE Pre-Participation Evaluation Form (PPE/Sports Physical). Each sports physical is only valid for 365 days in New Jersey. Please work to procure a Doctor’s Appointment to complete your child’s sports physical paperwork (Pre-Participation Evaluation Forms-PPE).
Step 3- Complete online and print the
NJDOE Updated Health History Updated Questionnaire and parents/guardians should complete the form. This form only requires parents/guardians signatures.

THE MIDDLE SCHOOLS ARE NOT STAFFED DURING THE SUMMER SO ALL PHYSICALS SHOULD BE DROPPED OFF AT UHS.
